We know that taking time off from work, school or other activities to visit the doctor can sometimes be an unwanted interruption in your daily life. We’ve tried to make this easier by providing convenient online tools to save you time.

Registered patients at our Scottsdale Family Practice office can access our unique Patient Portal to do many things: login to your own private and secure patient account, view/update parts of your personal medical history, request an appointment at our office, request a renewal of your current prescriptions, or view the results/status of your most recent lab tests and more. For patients transferring care to another medical provider: We will transfer your DiabeVita medical records to your new physician upon receipt of your authorization. Simply print and complete this form and fax or mail it to us. The form also includes a partial listing of other physicians in our area to consider.
